To become a student at City Seminary, register for classes, or apply for student assistance, you will need to fill out the required forms. These forms enable the school to manage the classes and provide for accurate transcripts of your work.
All of our forms are now available for filling out online. Click on the proper form and fill it out by typing the information into the various sections and fields. When you have finished, please click the submit button to send the form to us.
- If you are a new student, you must fill-out the "Application for Admission" and "Registration" forms. Our Registrar will contact you regarding your application.
- If you are a returning student, you only need to fill-out the "Registration" form.
- Both new and returning students who wish to apply for tuition scholarships must fill-out the "Financial Aid" form for each new semester.
We also have our forms in downloadable PDF (Acrobat) format for you to print out locally. You can then fill out the forms and mail them to us.
When you attend the Open House we will have all the forms necessary in printed form and you can fill them out there.
